All fees are non-transferable and non-refundable. Once payment has been made, no exchanges, credits, or refunds

will be provided for missed or unattended classes.

Consent & Agreement

I, the undersigned parent/guardian, give permission for my child to participate in the dance class. I confirm that the

information provided is correct and that I will notify the dance teacher of any changes.
